Grant Recipients 2014-2015 - $670,433

Arkansas Department of Health-BreastCare – Serving Boone, Carroll, Madison and Newton Counties

-BreastCare will provide free screening and diagnostic services for breast cancer to uninsured and Arkansan women ages 40-64.  Regional Care Coordinators provide case management and navigation assistance to BreastCare clients diagnosed with breast cancer.  (501) 661-2787  ($26,771)

Community Clinic – Serving Benton and Washington Counties

-The “We Are Aware” program provides a culturally competent continuum of care for low-income, uninsured patients in Northwest Arkansas through breast care education, screening, diagnostic and treatment support.  (479) 751-7417  ($90,000)

Hope Cancer Resources – Serving Benton and Washington Counties

-Hope Cancer Resources provides cancer support and education in Northwest Arkansas by focusing on access to support services with a focus on improving quality of life and lowering the incidence of cancer.  Genetic testing is provided by this program.  (479) 361-5847  ($45,000)

Madison County Health Coalition – Serving Madison County

-Madison County Health Coalition provides a navigator to advocate for breast cancer patients and educate others.  Program provides not only navigation services, but also gas cards, lymphedema supplies and coordinates support group meetings.  (479) 738-2612  ($50,000)

Mercy Breast Center – Serving Benton and Washington Counties

-Nurse navigator works with all breast cancer patients through their breast cancer journey.  The program provides gas cards, prosthesis, bras and post-op camisoles as well as screening mammograms. (479) 338-3340  ($60,000)

Mercy Fort Smith – Serving Crawford and Sebastian Counties

-The “Save A Life” program focuses on education, navigation services, community outreach program and working with diverse groups in the community to coordinate the access to a mobile mammography unit.  (479) 314-5264  ($42,671)

North Arkansas Partnership for Health Education – Serving Boone and Newton Counties

-“Living Healthy: Screenings for Life” program is designed to reach residents with a comprehensive education, outreach and awareness campaign to increase participation in breast health education classes, breast self-awareness training and access to treatment.  This program also provides breast screenings, diagnostic screenings and patient advocacy.  (870) 391-3367  ($90,000)

Skaggs Foundation – Stone and Taney Counties, MO

-“Health and Hope- Project Outreach” provides access to breast care and cancer treatment for at-risk women by offering financial assistance for women in need of screening mammograms, diagnostic testing and transportation to and from appointments.  A nurse navigator is also provided to help those diagnosed with breast cancer navigate their journey. The Merlin Foundation – Serving Carroll and Madison Counties

-This program increases the level of early detection to underserved women, brings awareness of breast health issues to all area residents as well as proves guidance and transportation funds to other medical resources as needed.  This program provides screening services, coordinates mobile mammography events, navigational assistance and gas cards.  (870) 553-2355  ($41,599)

The Women’s Center at Sparks --- Serving Crawford and Sebastian Counties

-The “Women’s Outreach Program” focuses on providing services for improved early detection through screenings and diagnostic testing and provides on community education.  (479) 709-6875  ($50,000)

Washington Regional Breast Navigator Program – Serving Benton and Washington Counties

-The breast health navigator offers specialized guidance and support to those recently diagnosed with breast cancer as well as recurrent breast cancer.  The program provides a valuable continuum of care by following patients through their breast cancer diagnosis and treatment and into survivorship, linking to financial and support resources and serving as a patient advocate.  (479) 521-8024  ($39,392)

Regional Cancer Support Home – Serving Benton and Washington Counties

-The “No Excuses Breast Cancer Outreach Program” exists to bridge gaps and link individuals with the health care and education they need.  The program provides screening and diagnostic assistance. (479) 271-2257  ($90,000)
