Community Clinic- Serving Benton and Washington Counties $103,267
The “We Are Aware” program provides a culturally competent continuum of care for low-income, uninsured patients in Northwest Arkansas through breast care education, screening, diagnostic and treatment support.  (479) 751-7417

Donald W. Reynolds Cancer Support Home- Serving Crawford and Sebastian Counties $18,000
“To Save A Life-Survivorship” program provides breast cancer patients with support, advocacy, information, financial assistance and medical supplies such as lymphedema sleeves and mastectomy supplies.  The program also offers patient navigation services and breast cancer support groups.  (479) 782-6302

Hope Cancer Resources- Serving Benton and Washington Counties $76,195
Hope Cancer Resources provides cancer support and education in Northwest Arkansas by focusing on access to support services with a focus on improving quality of life.  The program covers genetic testing,  housing, prescription and travel assistance.  (479) 361-5847

Madison County Health Coalition-Serving Madison County $49,923
Madison County Health Coalition provides a navigator to advocate for breast cancer patients and educate the community on breast health.  The program also provides gas cards, lymphedema supplies and coordinates support group meetings.  (479) 738-2612

Mercy Breast Center- Serving Benton and Washington Counties $53,608
The Mercy Breast Center’s breast health program provides a nurse navigator works with all breast cancer patients through their breast cancer journey.  The program provides gas cards and diagnostic services.  (479) 338-3340

Mercy Fort Smith- Serving Crawford and Sebastian Counties $42,007
The “Save A Life” program provides education, navigation services, community outreach programs and works with diverse groups in the community to coordinate access to screening services.  (479) 314-5264

North Arkansas Partnership for Health Education- Serving Boone, Carroll and Newton Counties $121,000
“Living Healthy: Screenings for Life” program is designed to reach residents with a comprehensive education, outreach and awareness campaign to increase participation in breast health education classes, breast self-awareness training and access to treatment.  This program also provides screening and diagnostic services, as well as  patient advocacy and assistance. River Valley Primary Care Services-Serving Crawford and Sebastian Counties  $60,000
The River Valley Primary Care Services program provides support and assistance with screening and diagnostic services for patients in the River Valley.  The program also coordinates access to mobile mammography units and navigating patients through the health care system.  (479) 785-5700

Skaggs Foundation- Serving Stone and Taney Counties in Missouri  $45,000
“Health and Hope” provides access to breast care and breast cancer treatment through financial assistance for women in need of screening mammograms, diagnostic testing and transportation to appointments.  A nurse navigator is also provided to help those diagnosed with breast cancer navigate their journey.  (417) 348-8912

Washington Regional Cancer Support Home- Serving Benton and Washington Counties $156,000
The “Breast Health Program” provides a breast health navigator and a bilingual navigator who offers specialized guidance and support to patients on their breast cancer journey.  The program guides patients through diagnosis and treatment and into survivorship and links patients to financial and support resources as needed.  The program also provides screenings, diagnostic services and treatment support.  (479) 521-8024
